Over
400 families are homeless in Worcester each year. The
homeless families team, including bilingual Spanish-speaking
family advocates, doctors, nurses, psychologists, and
counselors, work together with families to identify their
needs and support their strengths. Our model of care delivers
needed health and social services, while supporting families
in achieving the highest level of self-sufficiency possible.
Services
include a team approach to:
- Primary health care for adults and children
- Family-centered advocacy and outreach
- Assessment of parents' needs, and children's behavioral
and emotional health
- Clinic-based, shelter-based, and home-based parent education
and support
- Parent groups
- Mental health services
- Transportation
- Information and referral to community agencies
- Shelter and home visits
- School and educational advocacy
- Housing advocacy
