What’s a Medical Record and What’s in it?

Family Health Center creates a medical record for all of our patients. A medical record is a folder, set of folders, or electronic files that have information about illnesses you have or had, treatment you received from you medical provider(s), details about your lifestyle (such as whether you are a smoker, etc.), and information about your family medical history. Also, your medical record has laboratory and imaging test results, medication that were prescribed to you , and reports or surgeries and other medical procedures. Your records could also include information about your participation in research projects, if any, along with forms you have completed by.

Our medical record staff is fluent in several languages; interpretation service for other languages is available.
