Care Coordination

The Care Coordination Program is a free program for adults ages 40-64 who live in Massachusetts. While the program will focus on low income, uninsured and under-insured adults ages 40-64, both men and women under age 40 and over age 64 may be eligible for enrollment.

The Care Coordination model includes support services that help reduce barriers to healthcare. By being a part of our program, you will have a team of people to help you with your health needs, and get the information, tests and care you need to be healthy.
