At Family Health Center, patients who have a chronic health condition, such as diabetes, hypertension and asthma, are offered Disease Management services by a team of skilled health professionals. The team consists of a specially trained Disease Management Nurse and two Case Managers, who work in close consultation with the patient's primary care provider, to provide patient education and support the patient in making lifestyle changes that are needed to successfully manage their chronic condition and maintain optimum health. As with all services at the health center, Disease Management Services are offered in a respecful and culturally competent manner. The three-person Disease Management Team offers services in English, Spanish and Portuguese. Other interpretation services are available onsite, when needed. The ultimate goal of the program is to improve the patient's quality of life.
